Heathrow tunnel collapse

The Health & Safety Executive inquiry into the 1994 tunnel collapse found that the public and workers were exposed to ‘grave risk of injury’ and only by remarkable good fortune was none hurt. The report states: ‘The collapse could have been prevented but a cultural mindset focused attention on the apparent economies [of the method used] and the need for production rather than the particular risks.’
